Use this form to request pet sitting. This is not a confirmed booking. Do not send payment until Salt Lake Paws approves the request and sends deposit instructions.

Client Information

This is a request form only. Submitting this form does not confirm or reserve pet sitting dates.

Requested Dates

Use the first date care is requested and the last date care is requested. The estimate uses a simple date range.

Example: flight leaves early, returning late, preferred first/last visit time, etc.

Requested Service Type

Choose the service type you are requesting. Non-overnight and overnight options are separate. Final approval depends on availability, pet needs, safety, and required records.

Estimated at the configured midday drop-in rate.

Pet Types & Pet Count

Accurate pet counts, species, breeds, ages, and care needs are required.

Include every pet in the home, even if care seems minimal.

Medical, Medication & Records

Disclose all medical, medication, behavior, and record-related needs before the sit begins.

Include dose timing, handling concerns, allergies, special diets, seizure history, mobility issues, etc.

Request-Only, Approval & Payment Policy

Request Only — Not a Confirmed Booking *

Client understands that submitting this form is only a pet sitting request. It does not confirm service, reserve dates, or create a booking.

Salt Lake Paws will review the request and may approve it, deny it, request changes, require additional information, or provide a revised estimate based on availability, care complexity, travel, safety, medical needs, behavior, records, access, holidays, or other factors.

Do Not Send Payment Until Approved *

Client understands they should not send any payment until Salt Lake Paws approves the request and sends written instructions with the approved total and required deposit amount.

Payments sent before approval do not guarantee a booking and may be returned or handled only at Salt Lake Paws' discretion.

Dates Are Not Held Until Deposit Is Received *

Client understands that even if a sit is approved, dates are not confirmed or held until the required deposit is received and Salt Lake Paws confirms the booking.

If another client books and submits the required deposit first, the requested dates may become unavailable. Approval of a request does not guarantee the dates unless and until the deposit is paid and Salt Lake Paws confirms the booking.

Non-Refundable Deposit *

Client understands that a 50% non-refundable deposit, or other deposit amount stated by Salt Lake Paws, may be required to secure approved pet sitting reservations.

Once paid, the deposit is non-refundable for any reason, including cancellation, travel changes, illness, weather, schedule changes, failure to provide records, failure to provide access, change of plans, or Client deciding not to proceed.

The deposit compensates Salt Lake Paws for reserving time, declining other booking opportunities, planning care, and preparing for the sit.

Pricing, Highest-Tier Billing & Estimate Policy

Highest-Tier Care Pricing Policy *

Client understands that pet sitting is billed based on the highest level of care required during the reservation period.

If any portion of the booking includes higher-complexity care, including but not limited to exotic animals, specialty handling, medication needs, increased supervision, young pets, behavioral concerns, or complex household requirements, the entire service period may be billed at the applicable higher service tier rate.

When exotic or specialty animals are included, pricing is based on the exotic pet sitting rate structure. The base exotic pet sitting rate includes up to two total pets regardless of type. Additional pets are billed as add-ons according to their category.

Example: if a booking includes one snake, one dog, and one cat, the sit is billed at the exotic base rate because an exotic animal is included. The base rate covers two total pets, and the third pet is billed as an additional dog/cat pet unless the additional pet is also exotic.

Final pricing is determined by Salt Lake Paws after review.

Estimate Only — Final Price May Change *

Client understands that the calculator provides an estimate only. The final total and required deposit may change after Salt Lake Paws reviews the request.

Final pricing may increase based on number of pets, pet type, exotic pets, young pets, medication, medical needs, behavior, breed/size considerations, cleaning needs, travel, holidays, access complexity, length of stay, special care, safety concerns, inaccurate information, incomplete information, or undisclosed needs.

Salt Lake Paws will provide final pricing and deposit instructions only after review and approval.

Pets Under 2 Years Old Surcharge *

Client understands that pets under 2 years old generally require additional supervision, management, cleanup, patience, and safety consideration.

A young pet surcharge applies for each pet under 2 years old, per day/night of service. Failure to disclose a pet's age accurately may be treated as misrepresentation and may result in revised pricing, refusal of service, termination of the sit, additional charges, or deposit forfeiture.

Holiday and High-Demand Date Surcharges *

Client understands that holiday or high-demand dates may include additional surcharges and limited availability.

If requested dates include holidays or high-demand periods, Salt Lake Paws may adjust the estimate before approval. Holiday surcharges are non-discountable and may be required in addition to the pet sitting deposit.

Exotic & Specialty Animal Disclosure

Exotic & Specialty Animal Liability Disclosure *

Client acknowledges that exotic, specialty, and non-traditional pets, including but not limited to reptiles, amphibians, birds, small mammals, fish, invertebrates, and other specialty animals, may not be covered under standard pet care insurance policies.

By requesting care for exotic or specialty animals, Client assumes full responsibility and liability for any injury, illness, escape, loss, death, property damage, environmental issue, enclosure issue, temperature/humidity issue, feeding issue, handling issue, or other incident involving these animals, except in cases of gross negligence or willful misconduct.

Client agrees that Salt Lake Paws shall not be held liable for outcomes related to exotic or specialty pet care beyond reasonable professional standards.

Client acknowledges that Salt Lake Paws has experience and training with a variety of animal species, including professional experience in zoological and specialized care environments. However, inherent risks remain due to the unique nature, husbandry requirements, sensitivity, and care needs of exotic and specialty animals.

Client agrees to provide complete and accurate enclosure, feeding, temperature, humidity, lighting, handling, safety, veterinary, and emergency instructions. Client accepts full responsibility for undisclosed risks, inaccurate instructions, enclosure failures, supply issues, or conditions outside Salt Lake Paws' control.

Non-Continuous Care Acknowledgment

Pet Sitting Is Not Continuous 24/7 Care *

Client understands that pet sitting is not continuous 24-hour supervision unless specifically agreed to in writing.

Pets may be left unattended for reasonable periods while Salt Lake Paws provides services to other clients, travels between appointments, rests, eats, handles errands, manages emergencies, or completes other business obligations.

Client is responsible for disclosing if any pet cannot safely be left alone for normal periods. This includes separation anxiety, confinement issues, destructive behavior, escape risk, medical monitoring needs, toileting needs, seizure concerns, geriatric needs, puppy/young pet needs, exotic husbandry needs, or any condition requiring unusually frequent or continuous supervision.

Failure to disclose that a pet cannot safely be left alone may result in refusal, cancellation, termination of the sit, deposit forfeiture, additional fees, or a revised service agreement.

Misrepresentation, Undisclosed Pets & Undisclosed Conditions

Misrepresentation Ends the Sit and Forfeits Deposit *

Client understands that accurate disclosure is required before services begin.

If Client misrepresents, omits, or fails to disclose the number of pets, pet type, breed, age, size, temperament, behavior, bite history, aggression, reactivity, escape tendency, resource guarding, medication needs, medical conditions, special care needs, vaccination/record status, access issues, household conditions, exotic animal requirements, or any other material information, Salt Lake Paws may immediately refuse, cancel, or terminate the sit.

If service is terminated for misrepresentation or undisclosed information, the deposit is forfeited. Client is responsible for arranging alternate care immediately.

Salt Lake Paws may, at its sole discretion, offer to restart the booking process using accurate information. Any restarted sit requires a new estimate, new approval, and a new deposit based on the corrected total due.

Undisclosed Complexity and Care Requirements *

Client understands that undisclosed complexity may include medication, injections, medical monitoring, intensive cleanup, separation anxiety, destructive behavior, escape risk, specialty feeding, exotic husbandry, temperature/humidity needs, aggressive behavior, household hazards, or care needs beyond what was requested.

Undisclosed complexity may result in revised pricing, additional fees, refusal of service, termination of service, or deposit forfeiture.

Third-Party Access & Property Rules

No Undisclosed People on Property *

Client understands that Salt Lake Paws does not share responsibility for pet care, property access, or household conditions with undisclosed third parties.

No friends, family members, roommates, houseguests, neighbors, cleaners, contractors, maintenance workers, other pet sitters, dog walkers, or any other third parties may stay at, enter, access, or use the property during the pet sit unless disclosed and approved in writing by Salt Lake Paws before the sit begins.

Undisclosed third-party access creates safety, liability, security, privacy, and pet-care risks. If undisclosed people are present, staying at the home, entering the home, or otherwise accessing the property, Salt Lake Paws may immediately cancel or terminate service and the deposit may be forfeited.

Client is responsible for notifying Salt Lake Paws in advance of any expected person, worker, cleaner, family member, visitor, delivery access, or property activity during the sit.

Home Access, Keys, Codes & Property Conditions *

Client is responsible for providing safe, accurate, and reliable access, including keys, lockboxes, door codes, garage codes, gate codes, alarm instructions, parking instructions, building access, pet location details, and any other information required for safe service.

Salt Lake Paws is not liable for service delays, missed service, lockouts, alarm issues, faulty locks, malfunctioning doors, broken gates, inaccurate codes, third-party access, roommates, cleaners, contractors, visitors, neighbors, building staff, pre-existing property conditions, or household hazards.

Client is responsible for securing valuables, sensitive areas, medications, chemicals, fragile items, weapons, hazardous materials, and restricted areas before services begin.

Records, Veterinary Authorization & Safety Termination

Required Records and Care Information *

Client agrees to provide all required vaccination records, health records, care instructions, access instructions, feeding details, medication instructions, emergency contacts, veterinary information, exotic care instructions, and other requested information before the sit begins.

Failure to provide required records or care information before the sit begins may result in denial, cancellation, or termination of service and deposit forfeiture.

Veterinary Authorization & Emergency Care *

In the event of injury, illness, suspected illness, medical distress, behavioral emergency, escape recovery, exotic husbandry emergency, or other urgent situation, Client authorizes Salt Lake Paws to seek veterinary care from the pet's primary veterinarian, preferred clinic, nearest available veterinary clinic, emergency veterinary provider, exotic veterinarian, or other appropriate animal care professional.

Salt Lake Paws will make reasonable efforts to contact Client and/or Client's emergency contact as soon as possible. Client understands that emergency situations may require immediate action before Client or the emergency contact can be reached.

Client is financially responsible for all veterinary fees, exams, diagnostics, medications, treatments, surgeries, hospitalization, emergency care, transportation, aftercare, and related costs.

Safety Termination Rights *

Salt Lake Paws may refuse, pause, modify, shorten, or terminate a pet sit at any time if safety, access, behavior, health, medical, environmental, property, communication, exotic care, or undisclosed condition concerns make service unsafe, impractical, inappropriate, or outside the agreed terms.

This includes but is not limited to aggressive behavior, undisclosed bite history, unsafe home conditions, inaccessible pets, lack of supplies, medication not disclosed, records not provided, third-party access, unsafe equipment, contagious illness, excessive reactivity, destructive behavior, escape risk, severe anxiety, unsafe enclosures, exotic husbandry concerns, or any condition that places pets, people, property, or Salt Lake Paws at risk.

If service is terminated for safety, misrepresentation, lack of records, undisclosed conditions, or violation of policy, Client remains responsible for arranging alternate care and the deposit may be forfeited.

Liability, Indemnification & No Guarantee

Liability Release & Indemnification *

Client acknowledges and agrees that Salt Lake Paws, including its owners, employees, contractors, helpers, representatives, and assigns, shall not be held liable for any injury, illness, escape, loss, condition, behavioral change, emotional distress, property damage, veterinary expense, exotic animal outcome, or other damage that occurs during the normal course of care unless caused by gross negligence or willful misconduct.

Client agrees to indemnify, defend, and hold harmless Salt Lake Paws from claims, damages, costs, expenses, veterinary bills, attorney fees, liabilities, or losses arising from the actions, behavior, health, medical condition, temperament, handling needs, undisclosed information, exotic animal needs, equipment, enclosures, or property of Client's pet(s), including injury to people, animals, staff, or property damage.

Client understands that animals can behave unpredictably even with reasonable care, professional judgment, and safety precautions.

No Guarantee of Outcomes *

Salt Lake Paws makes no guarantees regarding behavior, training progress, socialization, fitness, weight loss, temperament, pack compatibility, off-leash reliability, anxiety reduction, leash manners, exotic animal outcomes, medical outcomes, or any other outcome.

Services are provided using reasonable care, professional judgment, and good-faith effort based on the information available at the time of service.

Client understands the deposit is non-refundable once paid, dates are not held until the deposit is received, and service may be refused or terminated for misrepresentation, undisclosed conditions, missing records, third-party access, safety concerns, exotic care concerns, or violation of Salt Lake Paws policy.

Client agrees to be bound by the acknowledgments and terms in this request form.
