Salt Lake Paws Pet Care Agreement & Liability Waiver

Please review and complete this agreement before services begin. Each section is part of the agreement and requires acknowledgment.

Client Information

Pet Health, Vaccination & Behavior Disclosure

Salt Lake Paws relies on complete and honest disclosure to make safe service decisions.

This includes legally required and commonly recommended vaccines such as Rabies, Bordetella, DHPP/Distemper/Parvo as applicable.

Health, Vaccination & Behavior Disclosure Agreement *

Client certifies that all information provided to Salt Lake Paws regarding the pet's health, behavior, training, handling needs, bite history, aggression history, reactivity, escape risk, resource guarding, medical needs, allergies, medication, and vaccination status is accurate, complete, and current.

Client certifies that all pets receiving applicable services are current on legally required vaccinations and commonly recommended vaccinations, including Rabies, Bordetella, and DHPP/Distemper/Parvo, unless otherwise disclosed in writing and accepted by Salt Lake Paws.

Client agrees to immediately notify Salt Lake Paws of any contagious illness, coughing, vomiting, diarrhea, lethargy, limping, injury, parasite concern, skin issue, behavioral change, bite incident, aggression incident, escape attempt, or other health or safety concern before services occur.

Salt Lake Paws may refuse, pause, modify, shorten, or terminate services if a pet appears ill, injured, contagious, unsafe, excessively fearful, aggressive, reactive, or otherwise unsuitable for the scheduled service.

Client understands that failure to disclose relevant health, behavior, vaccination, or handling information may result in immediate termination of services and may place full financial and legal responsibility on Client for any resulting injury, damage, veterinary cost, claim, loss, or expense.

Assumption of Risk

Assumption of Risk *

Client acknowledges that pet care services provided by Salt Lake Paws, including but not limited to neighborhood walks, pack walks, adventure outings, pet sitting, drop-in care, pet taxi, transportation, feeding, medication assistance, and home access, carry inherent and unavoidable risks.

These risks include, but are not limited to:

  • Dog-to-dog interactions
  • Unpredictable animal behavior
  • Bites, scratches, pulling, lunging, jumping, slipping, tripping, or falling
  • Wildlife encounters
  • Uneven terrain, rocks, sand, gravel, mud, snow, ice, water, and pavement
  • Weather exposure including heat, cold, rain, snow, lightning, smoke, poor air quality, and wind
  • Vehicle travel, loading, unloading, and traffic conditions
  • Equipment failure
  • Escape attempts
  • Stress, anxiety, illness, injury, or aggravation of pre-existing conditions
  • Natural outdoor hazards and environmental exposure

Client understands that even with reasonable care, professional judgment, and safety precautions, all risks cannot be eliminated. Client voluntarily assumes all known and unknown risks associated with Salt Lake Paws services.

Pack Walk & Group Interaction

Pack Walk & Group Interaction *

Client acknowledges that pack walks, group walks, group activities, and any service involving dogs from different households involve additional risk.

Even well-socialized dogs may behave unpredictably. Minor scuffles, scratches, leash tangles, incidental contact, fear responses, resource guarding, overstimulation, or behavioral changes may occur despite supervision.

Salt Lake Paws selects compatible dogs using professional judgment but cannot guarantee absolute behavioral control, compatibility, or prevention of all incidents.

Client assumes all inherent risks associated with pack walks and group activities. Salt Lake Paws may approve, deny, remove, separate, downgrade to private service, or discontinue any dog from group settings at its sole discretion and without refund if safety concerns arise.

Client certifies that their dog has no undisclosed history of aggression, biting, severe resource guarding, or dangerous behavior and is suitable for group activity unless otherwise disclosed in writing and accepted by Salt Lake Paws.

Off-Leash Authorization & Discretion

Off-Leash Authorization & Discretion *

If Client authorizes off-leash activity, Client acknowledges that off-leash activity involves increased risk, including but not limited to escape, wildlife pursuit, injury, environmental hazards, interaction with people or animals, and delayed recall.

Salt Lake Paws retains sole discretion to approve, restrict, deny, or revoke off-leash privileges at any time based on safety, behavior, environment, trail conditions, leash laws, wildlife, other people or animals present, weather, professional judgment, or any other concern.

Authorization does not guarantee off-leash activity during every outing. Salt Lake Paws may require leash use at any time for any reason. Salt Lake Paws follows applicable leash laws and may decline off-leash activity even when Client has authorized it.

Client assumes all risks associated with authorized off-leash activity unless an incident is caused by gross negligence or willful misconduct.

Adventure Outings, Walks & Environmental Exposure

Adventure Outings, Walks & Environmental Exposure *

Salt Lake Paws services, including neighborhood walks, pack walks, adventure outings, hikes, pet taxi, and outdoor enrichment, may involve exposure to outdoor environments and natural conditions.

Client acknowledges that during any service, pets may be exposed to environmental elements including, but not limited to:

  • Dirt
  • Mud
  • Snow
  • Ice
  • Water
  • Sand
  • Grass
  • Gravel
  • Pavement debris
  • Brush
  • Brambles
  • Sticks
  • Burrs
  • Foxtails
  • Seeds
  • Plant material
  • Natural debris
  • Insects
  • Ticks
  • Fleas
  • Parasites
  • Wildlife
  • Weather
  • Heat
  • Cold
  • Smoke or poor air quality

Pets may return home dirty, wet, dusty, muddy, tired, or with debris. Salt Lake Paws does not provide bathing, grooming, brushing, dematting, nail trimming, cosmetic cleaning, or full debris removal.

Salt Lake Paws may perform a brief visual inspection and remove visible debris when reasonably possible. Complete detection or removal of debris, including foxtails, burrs, ticks, or plant material, is not guaranteed.

Client is responsible for performing a full inspection after services. Normal outdoor effects such as dirt, mud, moisture, debris, odor, minor superficial scratches, paw irritation, tiredness, or environmental exposure are not grounds for refund or liability.

Veterinary Authorization & Emergency Care

Veterinary Authorization & Emergency Care *

In the event of injury, illness, suspected illness, medical distress, behavioral emergency, escape recovery, or other urgent situation, Client authorizes Salt Lake Paws to seek veterinary care from the pet's primary veterinarian, preferred clinic, nearest available veterinary clinic, emergency veterinary provider, or other appropriate animal care professional.

Salt Lake Paws will make reasonable efforts to contact Client and/or Client's emergency contact as soon as possible. Client understands that emergency situations may require immediate action before Client or the emergency contact can be reached.

Client is financially responsible for all veterinary fees, exams, diagnostics, medications, treatments, surgeries, hospitalization, emergency care, transportation, aftercare, and related costs.

Salt Lake Paws is not responsible for veterinary costs unless caused by gross negligence or willful misconduct. Client authorizes Salt Lake Paws to use professional judgment in seeking care when needed.

Transportation & Pet Taxi

Transportation & Pet Taxi *

Client authorizes Salt Lake Paws to transport pet(s) when transportation is part of the requested or approved service, including but not limited to adventure outings, pet taxi, veterinary transport, emergency transport, trail access, park access, or other service-related travel.

Client acknowledges that transport may involve risks including vehicle movement, sudden stops, traffic delays, loading and unloading, anxiety, motion sickness, escape attempts, vehicle accidents, equipment failure, and environmental conditions.

Pets may be secured using leashes, harnesses, crates, carriers, seat restraints, vehicle barriers, or other reasonable safety methods selected by Salt Lake Paws. Salt Lake Paws uses reasonable safety measures; however, transportation risks cannot be eliminated.

Client assumes risks associated with transport unless an incident is caused by gross negligence or willful misconduct.

Equipment Responsibility

Equipment Responsibility *

Client is responsible for providing safe, secure, properly fitted, and appropriate collars, harnesses, leashes, crates, carriers, medication containers, feeding supplies, and any other equipment required for pet care unless Salt Lake Paws agrees otherwise.

Salt Lake Paws may refuse, pause, modify, or shorten service if equipment appears unsafe, damaged, loose, improperly fitted, inappropriate for the pet, or likely to create risk.

Salt Lake Paws is not responsible for injury, escape, equipment failure, property damage, or incident caused by faulty, worn, loose, damaged, defective, inappropriate, or improperly fitted equipment supplied by Client.

Client agrees to promptly replace or repair unsafe equipment when requested.

Home Access, Keys, Codes & Property

Home Access, Keys, Codes & Property *

Client authorizes Salt Lake Paws to enter the home, yard, garage, vehicle access area, building, or property as needed to provide scheduled services.

Client is responsible for providing safe and reliable access, including working keys, lockboxes, door codes, garage codes, gate codes, alarm instructions, parking instructions, building access, concierge instructions, pet location details, and any other information required for safe service.

Salt Lake Paws is not liable for service delays, missed service, lockout, alarm issues, faulty locks, malfunctioning doors, broken gates, inaccurate codes, third-party access, roommates, family members, cleaners, contractors, visitors, neighbors, building staff, landlord activity, HOA restrictions, pre-existing property conditions, or household hazards.

Client agrees to notify Salt Lake Paws immediately of any changes to access instructions, alarm settings, keys, codes, household occupants, cleaning schedules, visitors, contractors, or property concerns.

Client is responsible for securing valuables, sensitive areas, cleaning chemicals, medications, hazardous items, and fragile property before services.

Weather, Heat, Air Quality & Safety Modifications

Weather, Heat, Air Quality & Safety Modifications *

Salt Lake Paws may modify, shorten, reschedule, substitute, pause, or cancel services due to heat, cold, lightning, storms, snow, ice, smoke, poor air quality, trail conditions, unsafe pavement, road conditions, illness, injury, behavior concerns, wildlife, unsafe environments, access issues, or any other safety concern.

A modified service may include a potty break, shaded walk, indoor enrichment, local park visit, pet-friendly indoor location, shorter route, reduced exertion, water break, or other safe alternative selected by Salt Lake Paws.

Safety decisions are made at Salt Lake Paws' sole discretion. Client understands that safety modifications are not grounds for refund when the scheduled service was reasonably attempted or an appropriate substitute service was provided.

Scheduling & Seasonal Time Blocks

Scheduling & Seasonal Time Blocks *
Salt Lake Paws operates using flexible time blocks rather than fixed appointment times. Services occur within time windows, not exact start times. Client preferences are considered but not guaranteed. Scheduling may shift due to route optimization, traffic, weather, safety, seasonal temperatures, daylight, client priority, pet needs, emergency situations, or multi-client coordination. Time block ranges may change throughout the year based on heat, weather, daylight, and safety needs.

Cancellation, Lockout & No-Show Policy

Cancellation, Lockout & No-Show Policy *

Client agrees to provide at least 24 hours notice for cancellations whenever possible.

Late cancellations may be charged up to the full scheduled service amount. No-shows, lockouts, inaccessible pets, missing or incorrect access instructions, unsafe conditions, failure to provide required equipment, or Client failure to provide access may be charged in full.

Salt Lake Paws may waive cancellation charges at its discretion for emergencies or unusual circumstances. Repeated cancellations, lockouts, access failures, or no-shows may require prepayment, deposits, schedule changes, or termination of services.

Payment Terms & Late Payments

Payment Terms & Late Payments *

Payment is due at the time of service unless prior billing arrangements have been approved in writing.

Monthly billing or invoicing may be available only by prior approval. Deposits or prepayment may be required for new clients, pet sitting, extended bookings, high-demand periods, or clients with late payment history.

Invoices are due within the stated payment window. Late payments may result in late fees, required prepayment, deposits for future services, paused services, cancellation of future bookings, or collection efforts.

Salt Lake Paws may refuse or pause future services for unpaid balances.

Holiday Surcharge Policy

Holiday Surcharge Policy *

Salt Lake Paws provides dependable, year-round care, including rain, shine, or snow. Because availability is limited and demand increases on certain holidays, a surcharge applies to services booked on those dates. Holiday surcharges are automatically added to invoices and are non-discountable.

  • Independence Day (July 4)
  • Pioneer Day (July 24)
  • Labor Day
  • Thanksgiving Day & Black Friday

Minor Holidays – $10 surcharge:

  • Martin Luther King Jr. Day

  • Juneteenth (June 19)
  • Veterans Day (November 11)
  • Indigenous Peoples' Day / Columbus Day
  • Valentine's Day
  • Halloween

Holiday availability is not guaranteed. Holiday bookings may require advance confirmation, deposits, or prepayment.

Pet Sitting Deposit Policy

Pet Sitting Deposit Policy *

To secure pet sitting reservations, a 50% non-refundable deposit may be required at the time of booking.

Requested dates are not confirmed or held until the deposit is received. Availability is not guaranteed without confirmation.

Pet sitting deposits compensate Salt Lake Paws for reserving time, declining other bookings, and preparing for care. Remaining balances are due according to the invoice or booking terms.

Service Refusal & Termination

Service Refusal & Termination *

Salt Lake Paws reserves the right to refuse, modify, pause, or terminate services at any time if safety, behavior, health, access, payment, communication, weather, environmental, or property concerns make service unsafe, impractical, inappropriate, or outside Salt Lake Paws' professional judgment.

This may include, but is not limited to, aggressive behavior, undisclosed bite history, unsafe home conditions, repeated non-payment, harassment, inaccurate information, inaccessible pets, unsafe equipment, contagious illness, excessive reactivity, severe anxiety, resource guarding, or any condition that places pets, people, property, or Salt Lake Paws at risk.

Salt Lake Paws may require private service, muzzling, veterinary clearance, training, updated equipment, prepayment, deposits, or revised service terms as a condition of continued service.

Photography, Social Media & Merchandise Release

These permissions are optional and may be declined without affecting pet care services.

This is specifically permission to use photos/videos of your pet on Salt Lake Paws merchandise or promotional products.

Media Release Acknowledgment *

Client understands that photography, social media, website, advertising, and merchandise permissions are optional.

If Client grants permission, Client authorizes Salt Lake Paws to use photos or videos of Client's pet for Salt Lake Paws social media, website, educational content, promotional materials, advertising, and merchandise, depending on the permissions selected above.

Client may decline permission without affecting pet care services. Client may revoke future permission in writing, but understands that prior uses may not always be fully removed from already published materials, printed materials, or distributed content.

Liability Release & Indemnification

Liability Release & Indemnification *

Client acknowledges and agrees that Salt Lake Paws, including its owners, employees, contractors, helpers, representatives, and assigns, shall not be held liable for any injury, illness, escape, loss, condition, behavioral change, emotional distress, property damage, veterinary expense, or other damage that occurs during the normal course of care unless caused by gross negligence or willful misconduct.

Client agrees to indemnify, defend, and hold harmless Salt Lake Paws from claims, damages, costs, expenses, veterinary bills, attorney fees, liabilities, or losses arising from the actions, behavior, health, medical condition, temperament, handling needs, undisclosed information, or equipment of Client's pet(s), including injury to people, animals, staff, or property damage.

Client understands that animals can behave unpredictably even with reasonable care, professional judgment, and safety precautions. Client remains responsible for damages caused by Client's pet(s), including bites, injuries, property damage, veterinary bills, third-party claims, and related costs.

No Guarantee of Outcomes

No Guarantee of Outcomes *

Salt Lake Paws makes no guarantees regarding behavior, training progress, socialization, fitness, weight loss, temperament, pack compatibility, off-leash reliability, anxiety reduction, leash manners, or any other outcome.

Services are provided using reasonable care, professional judgment, and good-faith effort based on the information available at the time of service.

Client understands that animals are individuals and outcomes may vary.

Entire Agreement, Severability & Governing Law

Entire Agreement, Severability & Governing Law *

This Agreement represents the entire understanding between Client and Salt Lake Paws regarding pet care services and supersedes prior oral or written discussions regarding the same subject matter.

If any part of this Agreement is found unenforceable, the remaining terms shall remain in effect to the fullest extent allowed by law.

This Agreement shall be governed by the laws of the State of Utah. Any legal action shall be brought in Salt Lake County, Utah unless otherwise required by law.
